Summary
Interviews patients in the Emergency Department for the purpose of obtaining and recording an accurate record of current medication use. Understands meaningful use as it pertains to medication reconciliation. If trained, under the direction of a Pharmacist or Pharmacy Manager, the Pharmacy Technician 2 assists to prepare, dispense and deliver all medications to patient care units, physician offices and clinics. Assists in the preparation of IV solutions and other administrative assignments. Is fully competent and fluent in a 797 compliant IV room and assists in the preparation of IV solutions.
Education
High School diploma or necessary education/training to obtain certification; or Pharmacy students entering into their second year of professional training.
Certification, Registration & Licensure
Must be registered in the state of NH. in addition, either CPhT certification or registration in NH as a pharmacy intern is required.
Experience
Three (3) years of experience working in a pharmacy setting within a healthcare environment or retail pharmacy. Experience should include patient interaction, interacting with other pharmacies, computer skills including data entry. Critical thinking, attention to detail and the ability to prioritize are essential skills required. Strong and effective communication skills with both patients and other members of the multi-disciplinary team.
Responsibilities
Interviews patient or their authorized agent for the purpose of obtaining a current medication history.
Performs medication data entry into appropriate systems based on patient medication list.
Collaborates with multi-disciplinary team for seamless patient flow during the admission process.
Educates and trains others on all processes related to taking a medication history.
Performs all aspects of pharmacy technician 2 role.
Performs job specific requirements and other duties as assigned.
Serves as a resource for education of all staff in medication history activities.
