We are currently seeking an experienced Social Work Director to provide services for our 120 bed skilled nursing facility. The qualified candidate must have strong interpersonal, written and organization skills. This position requires an individual who is empathetic, enthusiastic, creative, has the ability to work independently, and can communicate with people of diverse backgrounds.
Job Duties:
Job duties include, but are not limited to: conducting psychosocial assessments on new admissions, developing plans of care, and discharge planning. The social worker is responsible for timely resident evaluation and documentation of admission and quarterly assessments, and MDS completion, coordinating interdisciplinary care plans and discharge planning, investigating and documenting resident and care complaints.
