Job Title: Medical Social Worker - MSW - Geriatrics

Social Service Professionals, "the Professional's Choice" for Social Work and Therapy is looking for a Social Worker (MSW) to provide in-patient medical services to geriatrics patients at a hospital in Santa Barbra, CA. The MSW is expected to perform care coordination, resource linkage, clinical interventions, program development, utilization review, bio-psycho-social assessments, and discharge planning

Examples of Essential Duties:

Evaluate patients’ psycho-social needs;

Evaluate patients’ family/social situation to support a treatment plan;

Participate in on-going counseling;

Make recommendations to the healthcare team for appropriate psycho-social support;

Utilize community resources to assist patients’ psycho-social needs;

Document history, care plans and prepare reports as needed.

Qualifications:

Master’s degree in social work from a CSWE accredited School of Social Work.

Current AHA-approved CPR card

CCS - California Children Services (CCS) Paneling (must have in hand at start date)

Documentation System: EPIC (must be proficient)
