Client Care Coordinator / Quality Assurance

The Client Care Coordinator is responsible for all aspects of client management. This includes working closely with the HR Coordinator and Staffing Coordinator to make the best client/caregiver match, caregiver training, caregiver supervision, prospect client in-take, prospective client assessments, client visits and the development and maintenance of positive client relationships. We are looking for someone that loves working with people and exudes the people skills needed to interact positively and professionally with clients, their families and the caregivers that serve them.

This person also needs to enjoy working in a fast-paced environment, demonstrates the ability to think critically and be able to creatively and effectively solve problems.

The Care Coordinator Position requires the ability and willingness to work hours outside the normal 8 a.m. – 4:30 p.m.  office hours, when necessary.

Responsibilities Include (But are not limited to):

• Responsible for receiving and processing incoming Service Inquiry Calls/Convert calls into in-home assessment visits/Assessments to Client
• Responsible to Perform Client Assessments with prospective clients/Introduces CareOne Senior Care
• Responsible for Developing Individualized Client Center Care Plan
• Responsible for Client Satisfaction and Implementation of Care Plan

• Responsible for Establishing initial client record and completing all client onboarding requirements.
• Provides introduction of caregiver to client.
• Provides support, feedback and training to Caregivers.
• Responsible for reassessment of clients and quality assurance visits.
• This position will cross train with office duties, including scheduling and basic office duties.
• Responsible for taking On Call responsibilities a minimum of One (1) week night per week and one weekend per month.

The ideal candidate will possess the following:

• One year of related Care Coordinator Experience
• CNA certified with 2 years minimum experience.
• High School Diploma required. Associates degree or higher preferred.
• Outgoing, compassionate and positive personality.
• Ability to multitask, stay calm under pressure, work quickly and efficiently
• Exceptional attention to detail, organization, and ability to prioritize
• Strong time management, written, and verbal skills
• Problem solving, problem prevention, and issue resolution skills
• Computer literacy and program skills are important
