Intake Coordinator - Home Care evaluates patients referred to home care services and determine the types of services to be provided. Interviews the patient and family to determine appropriateness for services. Being an Intake Coordinator - Home Care obtains initial and ongoing referral documentation from physicians, insurers, and providers. Coordinates the development of patient home care plans that specify the frequency and duration of the services provided. Additionally, Intake Coordinator - Home Care initiates care with service providers. Utilizes health record management software. Requires a high school diploma or equivalent. Requires licensed practical nurse LPN credentials. Typically reports to a manager or head of a unit/department. Intake Coordinator - Home Care's years of experience requirement may be unspecified. Taylor Care Behavioral health is seeking an Intake/Operations Coordinator in Tuckerton, New Jersey. Seeking an energetic, flexible and reliable individual to work in a team-oriented environment serving clients in a Mental Health Adult Partial Care program.
Qualifications: Possess a master's degree in a mental health, rehabilitation or human services field, or a bachelor's degree and a minimum of two years experience in providing mental health services; or possess a relevant professional credential, such as: licensed clinical social worker, licensed professional counselor, licensed associate counselor, licensed rehabilitation counselor, licensed clinical alcohol and drug counselor, licensed psychologist, or master of science in nursing; or BA in a Human Services field, OR an AA in Human services field plus 2 years experience in Human Services, OR no degree and experience in Human Services.
Responsibilities include but are not limited to the following:
Duties:
1) Coordinates and Facilitates transportation, including picking up and dropping off clients as needed
2) Scheduling/hiring drivers, mapping transportation routes, training new staff and supervision of drivers
3) Responsible for scheduling vehicle maintenance
4) Maintains all associated records for transportation of clients
6) Administrative work as needed
7) Food service distribution supervision
8) Scheduling of and conducts the Intake process such as initial assessments and Prior Authorizations for services
9) Group scheduling and transportation scheduling of new clients
9) Clinical review and acceptance of potential clients in absence of Program Director
10) May hold caseload/conduct groups if needed
11) Assumes other responsibilities as assigned by Program Director, Executive Director and/or Medical Director/APN
