A Brief Overview
Responsible for support to Physical, Occupational, and Speech Therapy services administered to patients in their homes. As a full-time Physical therapist, treats patients of all ages, all types of disabilities and multiple medical problems. Evaluates care plans and administers physical therapy to patients in their own homes. Ability to apply appropriate body mechanics and use of assistive equipment in all patient transfers.
What you will do

Effectively assist Rehabilitation Staff by providing additional clinical support related to the therapy scope of practice including but not limited to identification of therapy skilled need and identification of progression towards goals.

Collaboration with the Director of Home Care to ensure criteria based performance evaluations are completed on a timely basis.

Participate in the interview process of candidates for employment and ensure staff qualifications are in compliance with State and Federal Requirements.

Collaboration with the Director of Home Care and Quality Team to identify and implement educational programs related to the Rehabilitation Staff.

Ensure that staff is updated and compliant with Regulatory and practice updates.

Collaborate as needed with the Intake Team to review rehab referrals and to assess appropriateness.

Collaborate as needed with Clinical Supervisors to ensure compliance and appropriate utilization of therapy services.

Oversight of rehab order compliance, reassessment, appropriate discharge planning, co-treatment, appropriate recertification and utilization aligned with the payment model.

Collaborate with Office Support Associates to ensure compliance and timely agency discharge process.

Completion of payroll, PTO approval/denial and weekend/on call coverage.

Oversight of Office Support Associate related to Rehabilitation scheduling, compliance with initial evaluations and productivity.

Collaborate with the Quality Team to develop Performance Improvement Programs and Policies.

Participate in Regulatory Compliance Surveys, as well as Rehab Corrective Action Plans and monitor performance as needed to ensure compliance with Regulatory Mandates.

Collaborate with the Interdisciplinary Team to ensure a comprehensive care approach in patient care.

Communicate to the Home Care Leadership Team all pertinent Rehabilitation concerns/issues.

Ensures staff competency by performing annual field competency visits, chart audits and identifying areas that need education and training focus.

Support field staff by performing joint visits for consultation on complicated cases and eligibility for Maintenance Programs.

Support field staff by assisting with clinical follow up as needed.

Maintain a partial patient caseload based on administrative duties.

Demonstrates compliance with the Hospital and Home Care dress code policy by appearing well groomed, wearing ID badge while on duty and maintaining a professional appearance.

Observes Hospital/Home Care policies concerning Fire and Safety regulations, Infection Control Standards, and other rules governing conduct while at work.

Demonstrates a commitment to own work schedule by adhering to attendance standards and obtaining coverage for cases during illness or vacations.

Provides care and services to reflect a commitment to the HNMC Mission Statement.

Represents hospital/department in community/department events.

Oversees peer review on documentation and observation of staff.

Completes all tasks in a timely manner. Adheres to all due dates.

Performs additional related duties as the need arises.
Education Qualifications

Master's Degree Required

DPT Preferred
Experience Qualifications

10 years At least 10 years of clinical experience Required

Minimum 3 years in a Home Health care setting Preferred
Knowledge, Skills, and Abilities

Knowledge of home exercise programs and equipment

Knowledge of Medicare, Medicaid and third party insurance reimbursement

Knowledge of State, Federal and JC standards for home care.
Licenses and Certifications
