Intake Coordinator - Home Care jobs in Evansville, IN
Intake Coordinator - Home Care evaluates patients referred to home care services and determine the types of services to be provided. Interviews the patient and family to determine appropriateness for services. Being an Intake Coordinator - Home Care obtains initial and ongoing referral documentation from physicians, insurers, and providers. Coordinates the development of patient home care plans that specify the frequency and duration of the services provided. Additionally, Intake Coordinator - Home Care initiates care with service providers. Utilizes health record management software. Requires a high school diploma or equivalent. Requires licensed practical nurse LPN credentials. Typically reports to a manager or head of a unit/department. Intake Coordinator - Home Care's years of experience requirement may be unspecified. Qualification and Education Requirements :- Degree in health care or related field preferred
- Current Driver's License & Reliable Transportation
- At least three (3) to five (5) years of experience in health care, preferably in palliative care operations
- Knowledge of corporate business management, governmental regulations, CHAP standards, and private payer practices
Essential Duties:- Direct all daily patient referral and intake operations including participating in the establishment and implementation of intake policies
- Maintain census, pending admissions, red flag, and care plan meetings on stand up board
- Ensure compliance with all state, federal and CHAP as applicable referral/intake regulatory requirements
- Establish and maintain positive working relationships with current and potential referral sources, build and monitor community and customer to enhance perceptions of InHome Connect as a high-quality provider of services
