Intake Coordinator - Home Care jobs in Fort Worth, TX

About Us:

Heaven at Home Senior Care is a leading private duty healthcare provider dedicated to delivering compassionate care to our community. We are currently seeking a motivated Licensed Vocational Nurse (LVN) to join our team in a dynamic role focused on client care with emphasis on the intake and admissions process as well as supervision of field staff and in-office staffing coordinators.

Responsibilities:

• Conduct admissions assessments for new clients, ensuring comprehensive evaluation and coordination of care plans.
• Perform supervisory visits to ensure quality of care and compliance with regulatory standards.
• Participate in on-call rotations to address urgent patient needs outside of regular business hours.
• Oversee staffing schedules, ensuring adequate coverage and adherence to policies and procedures.
• Provide guidance and supervision to both field staff and in-office personnel to maintain high standards of care delivery.

Requirements:

• Current Texas LVN License..
• Previous experience in admissions, supervisory roles, and staff supervision preferred.

Prior experience in home health, private duty home care, or assisted living is preferred, but not required.

• Strong communication and interpersonal skills.
• Ability to work effectively in a fast-paced environment and make critical decisions independently.
• Flexibility to participate in on-call rotations as required.
