Intake Coordinator - Home Care evaluates patients referred to home care services and determine the types of services to be provided. Interviews the patient and family to determine appropriateness for services. Being an Intake Coordinator - Home Care obtains initial and ongoing referral documentation from physicians, insurers, and providers. Coordinates the development of patient home care plans that specify the frequency and duration of the services provided. Additionally, Intake Coordinator - Home Care initiates care with service providers. Utilizes health record management software. Requires a high school diploma or equivalent. Position Summary
The Intake Coordinator is the first contact for new clients referred to Ryan White Case Management Services. This position conducts the initial screening with all new clients. In addition, this position conducts the bi-annual phone screening with all clients. The Intake Coordinator also assists clients in obtaining required documentation, as appropriate. This position is responsible for scheduling the first appointment for all new clients with a Care Coordinator and with a Medical Case Manager to complete their assessment process. This position also refers to the Care Coordinators and Medical Case Managers as appropriate when clients need additional assistance after the annual screening is completed.
Essential Functions and Responsibilities
Provide direct one-on-one and phone screening and care services
Obtain Authorization for Release of Information between HIVA and Medical Provider
Schedule and conduct intakes and bi-annual eligibility reviews
Schedule Medical Case Manager and Care Coordinator assessments as needed
Maintain up-to-date HIV knowledge; provide HIV education to clients
Maintain up-to-date and accurate client records
Participate in necessary meetings
Assist with all necessary reports
Participate in required training
Comply with all applicable standards, guidelines, laws & administrative rules
Other duties as defined by Director and Ryan White Principles and Standards
Required Qualifications
Two years of administrative experience or related work experience
Valid Oregon driver’s license, driving record sufficient to be covered by agency auto insurance policy, ability to transport self to job-related events, meetings and locations
TB test (provided at agency)
Excellent problem-solving skills
Experience maintaining confidential documents and files
Experience collecting and discussing sensitive material over the phone and in-person.
Ability to multitask
Ability to manage multi line phone system
Strong written and verbal communications
Ability to maintain boundaries and manage visitors to agency public spaces
Attention to detail
Experience with Word, Outlook and Excel preferred
Customer service experience
Ability to conduct interviews/intakes effectively over the phone.
Ability to pass agency criminal background check and a pre-employment drug screen
