Coordinates the development of patient home care plans that specify the frequency and duration of the services provided. Additionally, Intake Coordinator - Home Care initiates care with service providers. Utilizes health record management software. Requires a high school diploma or equivalent. Job Summary:
The Lead Intake Coordinator is located at Promise Care NJ LLC and is responsible for managing the Intake process, including, but not limited to: receiving referrals, assigning referrals, reviewing patient needs and care plans.
EDUCATIONAL REQUIREMENTS: RN/LPN w/ experience in home care, insurance guidelines and reimbursements, skilled needs vs unskilled needs, basic coding knowledge
EXPERIENCE: 2 years of Home Health experience in either direct patient care or management. Knowledge of community resources and of standard home health care third party eligibility, reimbursement and coverage criteria for all payers.
LICENSURE/CERTIFICATIONS: Licensed to practice as a Registered Nurse in the state of New Jersey or Licensed Practical Nurse . Must hold valid New Jersey drivers license.
