Intake Coordinator - Home Care evaluates patients referred to home care services and determine the types of services to be provided. Interviews the patient and family to determine appropriateness for services. Being an Intake Coordinator - Home Care obtains initial and ongoing referral documentation from physicians, insurers, and providers. Coordinates the development of patient home care plans that specify the frequency and duration of the services provided. Additionally, Intake Coordinator - Home Care initiates care with service providers. Utilizes health record management software. Requires a high school diploma or equivalent. Requires licensed practical nurse LPN credentials. Typically reports to a manager or head of a unit/department. Intake Coordinator - Home Care's years of experience requirement may be unspecified. As the voice of Angels of Care, the CCC acts as the main point of contact for our medically fragile client’s families. Duties include but are not limited to: receiving incoming calls and making outbound calls, assist with maintaining client/nurse schedules, developing positive relationships with nurses and families, working closely with clinical staff to provide the best care for our clients and maintaining a positive attitude. This is an hourly office position Monday through Friday from 9am to 5pm in our Pueblo office.
