Job Duties:

Acquire Client Consent forms from Clinical Staff team, ensure is 0 error on Client Files, physical and system upload is correct and free of errors. Assist in inputting data or company online trackers ensuring all tasks are matching with no errors. Overall checking if each incoming Client receive the proper clinical services, following up on exact visit dates from each clinical disciplines. Communicating all reports to each Client group.

Requesting documents from facilities, Physicians Office, etc. for each Client as necessary for us to continue care for each Client. Electronically Faxing all documents for each client to primary care and collecting documents that require physician signature in a timely manner, and uploading to our system. Ensuring all the documents are received and uploaded with free of error to our system.

Assisting Intake with Client Profiling and following up on assigned disciplines.

Other duties as assigned.

Job Type: Part-time

Pay: $16.00 - $20.00 per hour
