Job Description

Westminster-Canterbury on Chesapeake Bay is currently hiring a PRN Intake Coordinator - Home Health.

Full-Time

What will I do?

Our Home Health Intake Coordinator manages referrals for all home and community-based services programs. This position is responsible for accepting referrals and following up with client, family, facility or physician's office to obtain needed information. Other duties include, confirming Medicare and insurance eligibility, maintaining a referral log, and entering new clients into computer system.

What do I need for this role?

• High School Diploma or GED
• At least one (1) year of office experience in a healthcare setting, preferred
• Prior experience with Home Health and Hospice, preferred
• Prior experience in data entry and maintaining databases, preferred
• Proficiency in Microsoft Office Suite and Google Applications
• Demonstrated excellence in customer service skills, including telephone etiquette
• Must have valid state driver's license, proof of motor vehicle insurance, and a satisfactory driving record (for on-call duties)
