We are presently recruiting for an Intake Coordinator to join our Behavioral Health Team. This position will be based out of our Stratford location but will have responsibility for all Optimus sites. Bilingual in Spanish is preferred.
About this Position:
The Intake Coordinator’s primary responsibility is to conduct biopsychosocial evaluations for patients seeking behavioral health services through Optimus Health Care. Through this work, the Intake Coordinator will coordinate operations to schedule evaluations and work with the referral team to assist BH patients in coordinating initial evaluations. The Intake Coordinator will also provide clinical services to a modified panel of patients.
Admissions Evaluator responsibility
Provides screenings and intake services to patients and potential patients.
Conducts biopsychosocial assessments for patients seeking behavioral health services.
Documents evaluations, initial treatment plans, collateral contacts and crisis intervention provided appropriately.
Reviews all paperwork accompanying patients to ensure adherence with behavioral health and regulatory policies.
Supports clinical team as back up when needed.

JOB QUALIFICATIONS/REQUIREMENTS
EDUCATION: Master’s Degree in Social Work
EXPERIENCE: Two to four years related experience in behavioral health, particularly outpatient, preferred. Experience in completing assessments in an outpatient setting required.
LANGUAGE SKILLS: Bilingual (English/Spanish preferred)
LICENSURE / CERTIFICATION: LMSW or LCSW through State of Connecticut Department of Public Health
