Intake Coordinator conducts initial assessments of patients to determine mental health needs and establish care requirements. May assign patients to mental health practitioners based on intake assessments. Being an Intake Coordinator requires a bachelor's degree in a related area. Job description
Intake Duties
• Develops a plan for client arrival for intake in a timely manner
• Completes KIPU Intake documents with clients
• Handles the client during entire intake process, during transition, once decision is made to admit to MTR to their care being handled by their Counselor.
• Passes potential candidates to Counselor for clinical assessment for intake.
• Collects insurance information and discusses finances.
• Assists in verification of benefits and eligibility and may do intake as needed
• Coordinates with other disciplines to provide highest level of care
• Answers admission calls and screens residents for treatment
• Supports to remove logistical barriers to admissions
• Explores resident financial resources, bed availability and overall census management
• Manages a waiting list if needed
• Tracks leads, admissions, and discharges
• Communicates with referral sources to ensure the referent is promptly notified of admission and retrieve appropriate documents for admission (with resident’s written consent)
• Attends staff meetings
• Attends management meetings as requested
• Other duties as assigned (See BELOW)
Clinical Duties:
• Lead or assist with counseling groups as directed by clinical team.
• Document Clinical duties and observations
Program Duties:
• Drives as needed to appointments or outings
• Cooks and Cleans as needed
• Assist in supervising and training other staff when said staff are performing program duties.
Note: In all duties, all employees are to maintain resident Electronic Health Records (EHR) documents in the resident file per the APA ethical codes, organization policies, state and federal regulations, and The Joint Commission standards.
