JOB TITLE: Intake Coordinator

REPORTS TO: Administrator

IMMEDIATE SUBORDINATES: Operation Manager

Organizational Overview:

Golden Home Healthcare is a leading skilled home healthcare provider in Central Pennsylvania. We are known in the region for providing the highest quality of care you need. We are a locally owned and operated home health agency that offers skilled nursing services and home health aide services. We are recruiting for the key position of Central Intake Coordinator covering our Central, Pennsylvania facility. This role is a newly created position due to organizational growth and will be based out of corporate headquarters in Harrisburg, Pennsylvania.

Responsibilities:

• Drive the central intake / admissions process and support census growth for the agency.
• Work closely with our external marketing, school, and hospital liaisons.
• Provide members (prospective members, their families, and referral sources) with agency and program specific information.
• Evaluate and assess potential members for clinical appropriateness and placements into skilled nursing/home health programs.
• Help managing the admissions process and ensure proper admission documentation.
• Participate in agency training and agency specific sales and marketing team events and plans as needed.

Requirements:

• Prior experience in an Admissions Coordinator position in a Skilled Nursing Facility (SNF)/home healthcare, working experience with special needs population including IUs and Schools is preferred.
• Must have High School Diploma/GED, Associate degree or higher is preferred.
• Solid computer skills, including Microsoft Office products.
• Working knowledge of federal & state regulations and reimbursement (Managed Care).
