Position Summary: Intake Care Managers serve as the initial in-person contact with applicants who are newly referred to the State Home Care Program. They are responsible to conduct in-home initial assessments of these applicants living in the community to determine eligibility for the State Home Care Program and assist them in obtaining services and programs which will enhance their quality of life and allow them to live as independently as possible with dignity and respect.

Participation in Staff Development Programs:
Qualifications: Bachelors degree in social work, human services or a related field required. Associates degree with significant relevant work experience may be substituted for a portion of degree. A minimum of 1 year of experience working with elders and conducting in home assessments strongly preferred. Strong written, verbal and computer skills as well as excellent time management and organizational skills and the ability to work successfully within a fast-paced environment required. Valid Driver's License and reliable transportation required as field work is an essential component of the position. Bilingual capacity in Spanish is preferred.
Hours: 37.5 per week
