We are seeking an individual to join our team in the role of Intake Coordinator (LPN).
The Intake Coordinator is responsible for all facets of the intake and enrollment process including serving as a primary point of contact for referrals and performing assessments (physical, social, etc.)
This role also serves as a clinical resource to our program staff and provides routine and preventative nursing care (health monitoring, medication administration, vitals, health education, etc) and assists participants with ADL's and activities.

This is a full-time hourly position - 32-37.5 hours per week (flexible for right candidate). After orientation, required schedule will be 7:30AM start, Monday-Friday. We are closed on all major holidays - no evenings or weekends.
Starting rate of pay is $22-27 per hour depending on experience.
