Job Summary:

The intake coordinator assists in guiding residents through their healthcare admissions process.  Duties include ensuring the initial paperwork registering and admitting the patient to the medical facility is completed, including all patient medical and insurance information is obtained.  Data entry experience and a strong knowledge and understanding of health insurance is required.

RESPONSIBILITIES: 

• Receives, evaluates, and executes the initial resident intake process to assure patient verification and assessment occur in a timely fashion.
• Review all potential applications and submit to finance for approval.
• Completes checklist for all referrals.
• Coordinate admissions with medical, nursing, and other departments as well as the resident and family members.
• Complete all Medicaid recertifications in a timely manner and submit them to the appropriate DSS Office.
• Complete all Representative Payee forms and submit to the appropriate agencies.
• Prepare month end reporting for department reporting.
• Complete any requested/necessary personal fund accounts to include.
• Maintains facility admission waitlist.
• Update and disperse all resident rosters upon each admission and discharge.
• Update all resident forms (roughly 20 files) upon each admission and discharge that are used by nursing staff to provide care.
• Update (as needed) emergency contact lists for residents and their families for emergency purposes.
• Serves as a resident advocate and ensures resident rights are maintained.
• Serves as a resource for nursing staff.
• Is a liaison with residents, families, and other departments to adequately plan for and provide resident care.
• Ensures appropriate and ongoing communication to the resident, family, staff, providers, leadership, etc.
• Uses communication to enhance quality of resident care and staff working environment.
• Constantly strives to enhance the resident and staff experience.
• Immediately reports unusual problem/accident/incident regarding resident care to RN/House Supervisor.
• Promotes teamwork and collaboration among Mountainside staff members.
• Other job duties as assigned.

EXPERIENCE:

• Two (2) years clinical experience in long term care, required

EDUCATION:

• High school diploma or equivalent required, Bachelors Degree preferred

LICENSES/CERTIFICATIONS:  

• NYS LPN license, required
• NYS RN license, preferred
• BLS certification, required

OTHER: 

• Basic Computer Skills
• Subject to callback during emergency conditions
• Must have excellent customer service skills
• Must be able to work collaboratively
