    PURPOSE

    The Intake & Triage Nurse supports our interdisciplinary team by providing direct patient care and expertise across multiple locations, ensuring the delivery of high-quality hospice services to patients and their families.

    At Tri-Cities Chaplaincy, we carefully establish salary ranges for our positions by taking various factors into account, including education, certifications, knowledge, skills, experience, and market conditions. These ranges are tailored to match the diverse programs and services we offer. Our goal is to ensure fair compensation for candidates based on these factors while maintaining internal peer equity.


    Pay Range:$35.50/hr - $65.79/hr


    · Mileage Reimbursement: $0.67 per mile, for eligible positions.

    · Vacation: Accrual of hours each pay period based on hours worked.

    · Wellness Days: After the 90-day introductory period, eligible employees can convert up to two sick days into wellness days.

    · Annual "Yay Day": Full-time employees receive 1 paid day off every January to volunteer in the community.

    · Sick Time: Earned on an accrual basis, with a 90-day hold for new hires (usable after 90 days)

    · Paid Holidays: Ten recognized holidays with no waiting period for new hires.

    · Health Benefits: Opt-in for Low Deductible Health Benefits, Vision, and Dental starting the first month following your start date.

    · Health Benefit Options: Medical, Dental, Vision, Supplemental Life, Accidental Death and Dismemberment, GlobeLife Voluntary Benefits, and Life Flight.

    · Voluntary Supplemental Benefits: Pet Insurance, Globe Life, and Life Flight. 

    · Agency-covered benefits: Basic Life Insurance and Accidental Death and Dismemberment at 1x Annual Salary, Long-Term Disability, and Short-Term Disability.

    · 401k: You can start contributing right away and up to 5% employer matching after 1 year of employment for eligible employees.

    · Premium Pay: Bilingual in Spanish: Verified through ALTA staff testing and Approved Certifications: Premium pay upon review and approval.

    Requirements

    MINIMUM POSITION QUALIFICATIONS

    1. Valid Registered Nurse (RN) License in the State of Washington.
    2. Minimum of two years of experience in hospice, palliative care, or a related field.
    3. Demonstrated ability to adapt to different work environments and provide effective care in diverse settings.
    4. Excellent assessment, critical thinking, and problem-solving skills.
    5. Strong communication and interpersonal abilities to establish rapport with patients, families, and healthcare professionals.
    6. Ability to work independently, manage time effectively, and prioritize competing demands.
    7. Compassionate and empathetic approach to caring for patients and families during their end-of-life journey.
    8. Proficient computer skills and experience with electronic medical records systems.
    9. Must be flexible to work occasional evenings and weekends, as needed.
    10. This position is designated as safety-sensitive and will require pre-employment drug testing, which includes cannabis testing in WA State.

    CLINICAL STAFF POSITION QUALIFICATIONS

    1. As required per position/credentialing; successful completion of Blood Born Pathogens Training
    2. (Hospice House) Washington State Food Worker Card issued by the Department of Health

    RESPONSIBILITIES AND ESSENTIAL FUNCTIONS

    1. Provide compassionate and comprehensive nursing care to hospice patients in various settings including their homes, nursing facilities, and Hospice House.
    2. Serve as a primary point of contact for incoming referrals to the hospice and palliative care program,
    3. Coordinate the intake process, including documentation and admissions paperwork.
    4. Serve as a liaison between patients, families, healthcare providers, and community resources, ensuring effective communication and coordination of care.
    5. Complete admissions, assessments, and care plans in a timely manner.
    6. Provide telephonic triage coverage and care to evaluate patients’ needs and determine appropriate levels of care.
    7. Act as a resource for nursing personnel.
    8. Coordinate the schedule of the Hospice Admissions (Admit) Team and patient appointments.
    9. Develop and maintain relationships with key referral sources including but not limited to nursing homes, physicians, and hospital discharge planners.
    10. Manage facility and referral contact data.
    11. Maintain tracking and monitoring of referral source activity and satisfaction.
    12. Communicate with physicians, hospital staff, and patients regarding hospice and palliative care services.
    13. Conduct educational presentations on hospice and palliative care in the community, facilities, home health, and other hospice services.
    14. Attend community outreach functions and maintain market awareness.
    15. Explain the hospice and palliative care program accurately and thoroughly.
    16. Provide necessary admission information to the Intake and Medical Director.
    17. Assist in the creation and implementation of a marketing plan.
    18. Participate in regular team meetings, care conferences, and quality improvement initiatives to enhance the delivery of hospice and palliative care services.
    19. Meet with prospective patients, families, and referral sources.
    20. Work with Clinical Supervisor to ensure a smooth admission process.

    21. Identify opportunities for quality improvement and program development.

    22. Maintain accurate patient records and inform supervisors of issues.

    23. Input and maintain patient data in the computer system.

    24. Complete and conduct orientation and in-service training programs.

    25. Demonstrate familiarity with policies and regulations governing hospice care.

    26. Attend patient care conferences and contribute to discussions.

    27. Provide support during the bereavement process.

    28. Perform other duties as assigned.


    AGENCY REQUIREMENTS

    1. Demonstrate support and adherence to the agency’s Mission, Vision, and Values.
    2. Participate in community Education and Outreach to raise awareness of agency-provided programs including but not limited to hospice care, advanced care planning, and bereavement.
    3. Responsible for knowledge and compliance with agency policies, procedures, and practices.
    4. Responsible for maintaining and protecting Patient and agency information in a confidential manner. Practice appropriate HIPAA confidentiality by accessing patient-related information on a “need to know” basis only.
    5. Reliable personal transportation, and current and valid driver’s license and vehicle insurance.
    6. Satisfaction with state and agency requirements for Tuberculin testing 
    7. Hepatitis B immunization series, proof of series, or signed declination.
    8. Proof of applicable Vaccination(s) on file or approved accommodation/declination as appropriate on file.
    9. Must pass a criminal background screening and pre-employment drug screening.
    10. Ability to meet the physical demands of the position (see attached physical requirements and environmental conditions).
    11. Complete agency and departmental orientation and training program(s).
    12. Complete all continued learning, education, policy reviews, and required training by their respective deadlines.
    13. Maintain licensure(s) and certification(s) required for the position.
    14. Available to work as assigned/scheduled.
    15. Perform other duties as assigned.
