Position Overview :
Intake Coordinators welcome, register, and admit guests into our facilities. As an Intake Coordinator you will gather information about our guests and create guest records.
In this position, you will make appointments for guests and make arrangements for future medical treatments. Intake Coordinators are also responsible for answering incoming calls, verifying guest information, and performing clerical work, such as filing and faxing.
Ultimately, you will work with guests as they enter our facilities and aid other Intake Coordinators to ensure our guests are greeted and their medical information is organized.
Responsibilities :
Greet, process and work with our guests to register and get acclimated to our site.
Receive Intake referrals, research / create guest profiles, update / enter gathered information into the One System database and communicate this information to management.
Field questions from clients and responding to requests.
Complete incident reports as needed.
Monitor your assigned workstation within the facility.
Document Guest comings / goings, throughout the dorms / dining areas.
Intake and record accurate information on the Guest Master roster.
Read / explain Safe Navigation Center rules, policies, and procedures, to incoming guests.
Assign and record guests to dorm / bed, and lockers input information into the guests' weekly roster.
Maintain program guest weekly roster tracker, daily by imputing entries / exits of all guests, coordinate / record all bed moves, bed holds, and guests exits.
Sole responsibility for
Respond to client requests and escalate, as necessary.
Serve as on-site liaison with Guests, Case Managers, DPH, HSH, HSA, CBOs, and Vendors, via site phone, email, and in person.
Monitor / record all incoming calls on site phone. As a liaison you are required to remain professional, approachable, and courteous always.
Remember you are the initial contact of the Safe Navigation Center.
Maintain program guest weekly roster tracker, daily by imputing entries / exits of all guests, coordinate / record bed moves, bed holds, and guests exits.
Distribute weekly rosters to all Care Monitors, Shift Supervisors, and leadership / management team.
Receive and respond to Guest requests and escalate, as necessary.
Overseeing chores, meals and living space (rounds).
Conducting intakes / discharges.
Other duties as assigned.
Qualifications : Education :
Education :
High School diploma or GED
Experience :
At least 1 year of experience working with individuals and families in crisis, experiencing homelessness, substance use, and / or mental health issues.
Strong organizational and communication skills.
Ability to read, write, and understand the English language.
Customer service skills a must will be the face of the program and interact with both guest who have been referred for services and partner agencies, including outreach teams, other homeless providers, DPH, behavioral healthcare, etc.
Ability to engage with guests, BVHP partners and colleagues,
Maintain an empathetic demeanor in various situations and possess the de-escalate conflict whenever possible.
Ability and desire to work with diverse populations, possess cultural competencies and sensitivities and maintain appropriate boundaries with guests.
Ability to establish and maintain cooperative workplace relationships, other duties assigned.
Experience providing direct services to homeless populations preferred.
Physical Requirements :
Valid California Driver's License (if job functions require).
Able to meet required state, federal, local and BVHP standards (including, but not limited to) a comprehensive background screening and depending on the position, Live Scan fingerprinting clearance.
Ability to walk, sit, operate keyboards, tools, or controls and lift up to 40 pounds.
