Job Summary
The CPA Intake Coordinator is responsible for screening referrals, tracking potential placement options, and coordinating intakes from DFPS, Juvenile Probation, and other public/private entities.
Essential Duties:
Client Referrals:

Documents and maintains referral statistics for self-monitoring and future program planning.
- Responds immediately to all client referrals.

Screens and processes all referrals for placement and appropriately matches referrals to families equipped to meet the referred client's needs.

Participates as part of a multi-disciplinary team to determine best placement options for referred clients and obtains approval from CPMS/LCPAA staff for final placement decisions.

Completes and collects all required placement documentation prior to placement.

Coordinates the child’s placement into the home.
- Provides orientation to the child upon placement when necessary.

Enters all referral/placement information (client name, date of placement, SS#, PID#, Medicaid #, Level of Care, birthdate, etc.) within 24 hours of placement.

Maintains contact with homes waiting for children to be placed.
Home Intake/Updates/Closures:

Updates the Child Vacancy Database on a daily basis by 9 a.m.

Opens initially compliant homes in CLASS within one business day of request for verification.

Participates in matching meetings and advocates for families.

Receives notification of any home changes and makes appropriate updates in CLASS within one business day of notification.

Closes homes in CLASS within one business day of notification.
Education & Experience

Option 1 - Educational Qualifications A bachelor’s degree from an accredited college or university and nine credit hours in undergraduate level courses that focus on family and individual function and interaction. Professional Qualifications At least two years of documented full-time experience in a child-placing agency.

Option 2 - Educational Qualifications High school diploma (or GED equivalency). Professional Qualifications at least three years' of documented full-time experience in a child-placing agency.

Ability to effectively communicate with Pathways’ staff, foster-adopt parents, caregivers, support providers, external agencies, and professional service providers via phone, in person, and through written correspondence.

Competent in using Microsoft Word, Excel, Power Point, Go To Meeting, the Internet, and other software applications.

Organizational skills.

Ability to work independently and as part of a team.

Ability to effectively conduct public presentations in the community and with local service providers.

Knowledge of available community resources.

Proof of valid Texas Driver's License (Type C).

Three employment references.

Cleared criminal background check and signed statement regarding felony indictments/convictions.

Cleared TB test results (current within 12 months prior to employment).

Occasional reaching with hands and arms, stooping, kneeling, crouching, crawling, frequent sitting, standing and walking may be required for long periods of time and may involve climbing stairs, walking up inclines and on uneven terrain.
