Intake Coordinator/CSW

  • Purpose: This role provides direct care services and outreach to clients with Community Care Support Services (CSSS), coordinates with schedulers, and staff at every level. The primary focus of this role is to complete all intakes with incoming clients, scheduling the first appointments prior to the client leaving their initial intake. Ensure there is a heavy emphasis on guaranteeing the clients get to their appointments.

    Knowledge, Skills and Abilities:

    • CPSW, CSW to provide CCSS services in a Krossroads facility.
    • Implements agencies program for “Full Circle Continuity of Care and Parallel Service Models”, Choice Recovery Path, policies and procedures and follow office setting protocol.
    • Must be able to follow the “Comprehensive Community Recovery and Re-integration (CR2)” program for Justice Involved Services (JIS) referrals.
    • Support Christian based transitional housing setting recovery program.
    • Build a collaborative relationship with upper management and development program tracking tools, measure program efficacy, maintain outreach resources list and active referral list.
    • Collaborate on both micro and macro levels of agency quality assurance plan for Comprehensive Community Support Services (CCSS) caseworkers.
    • Support client CCSS services with all CSW, CPSW staff
    • Attend all team meetings and employ directives from immediate supervisor.
    • Must be able to navigate electronic health record system - Advanced MD, have excellent digital literacy, organization, and planning skills.

    Direct Services:

    • Verify insurance eligibility.
    • Collaborate with CR2 Director and Manager for legal referrals to coordinate appropriate services and support.
    • Understand and be to explain the CR2 requirements and expectations of the program.
    • Collaborate with the administrative team to schedule clients for intake services and warm hand off to assigned CSW.
    • Provide direct services to clients in a variety of settings for intake, screenings, UDS’s, vital signs and other services as assigned.
    • Ensure clients consents are signed and uploaded and notify immediate supervisor after completion of intake for further management.
    • Ensures client and staff safety.
    • Supports client's management.
    • Follows agency incident reporting protocols.
    • Coordinate wrap-around services in the community.
    • Refer clients to resources to support the 5 domains of health.
    • Coach clients to learn daily living skills thereby supporting a least restrictive care setting.
    • Provide, education, and support to maintain medication and other treatment regimens to both clients (e.g. a CSW does not handle medication directly).
    • May lead educational/skills groups for clients in areas such as basic skills, mental health, and addiction.
    • Utilizes Vocational Rehabilitation to support client’s employment goals.
    • Empowers each client to make his/her own life decisions.
    • Complete needs assessment, and updates as required.
    • Assists clients to formulate and achieve goals.
    • Participates in treatment team meetings.
    • Provides family education and support.
    • Documents DAP notes daily completing all required areas in a timely manner.
    • Complete additional documentation as needed including but not limited to crisis plan, CIR, emergent situation report notes, advanced directives, and relapse prevention plan.
    • Maintains healthy boundaries with the client, and others that may be involved in their lives.
    • Is physically able to complete job duties.
    • Proficiency in digital literacy including Microsoft Word, e-mail, telemedicine, and EHR (electronic health record) and guide team in these areas as needed.
    • Uses Seeking Safety curriculum as a tool to support client empowerment, safety, and insight to increase progress towards goals as applicable.
    • Other duties at the discretion of the leadership team.

    Qualifications:

    Must be 18 years of age or older and;

    • Have a CCSS certification or acquire this certification within 90 days or hire.
    • Preferred associate degree from an accredited university, and/or preferred experience in providing direct care to the client population.
