Intake Services Representative - Home Care coordinates and facilitates the intake process for home care patients. Provides information about home care services, eligibility criteria, and policies to patients and families. Being an Intake Services Representative - Home Care utilizes health record management and other software to process the flow of documentation required to schedule and deliver services to patients. Responds to questions and resolves issues to ensure customer satisfaction. Additionally, Intake Services Representative - Home Care requires a high school diploma or equivalent. Typically reports to a manager or head of a unit/department. The Intake Services Representative - Home Care works under moderate supervision. Gaining or has attained full proficiency in a specific area of discipline. To be an Intake Services Representative - Home Care typically requires 1-3 years of related experience.     This people leader is responsible for the performance of their department. Through coaching, directing, and, teaching through example, this individual ensures their direct reports are consistently delivering outcomes beyond expectation.

    Supervises: Individual contributors in Medical Records, Intake, and Authorizations

    POSITION RESPONSIBLITIES:
    • Develops and maintains an environment that inspires employee engagement by encouraging teamwork, showing appreciation, and providing support to achieve all stated goals.
    • Facilitates routine feedback sessions, one-on-one meetings, and functional training for the development of their team(s)
    • Inspires and leads change initiatives to ensure early and complete adoption.
    • Demonstrates the ability to pro-actively problem solve when next steps in a situation are unclear.
    • Develops, with urgency, an instructional level of expertise in the use a variety of systems, software, and applications.
    • Communicates effectively with staff, peers, and across Company teams.
    • Uses data to identify performance trends, celebrating wins and coaching staff on opportunities to improve; regularly recommends solutions to senior leadership.
    • Tracks and documents individual and team performance
    • Possesses exceptional time management and prioritization skills, actively managing the pace, quality, and quantity of workload execution in the department
    • Takes steps to identify and resolve conflicts in a timely manager, as they arise.
    • Contributes to staffing and workflow decisions.
    • Any duties or requests made by authorized leadership.

    POSITION REQUIREMENTS:
    • Intermediate to advanced Excel Skills preferred.
    • Minimum 2 years' experience in a team leader and/or Supervisor role
    • Proven track record of being able to achieve business goals, within deadlines, as an individual contributor and through others
    • Demonstrates effective communication and public speaking skills.
    • Able to deal professionally with company staff, referral sources (PCP), patients, etc.
    • Able to manage through diverse tasks and teams, navigating through multiple systems or environments efficiently and accurately.
    • Able to quickly adjusts to pace and volume changes in workflow.
    • Available to work evenings and weekends where a business need exists.
