Entry Level Qualifications:

• High School Diploma or GED, College preferred.
• Knowledge of chemical dependency desired.
• Two (2) years’ experience in business development or account management desired.
• Knowledge/skill of computer data entry is desired.
• Must be able to interact with the professional community.
• Excellent spoken and written communication skills.
• Must have reliable transportation.
• Be available for on-call schedule that may include a period of waiting to be engaged between 7am to 7pm.
• Must be self-motivated and able to work under minimal supervision.

General Responsibilities:

• Handle inquiries from prospective patients and area treatment centers, jails, DCS and Probation offices.
• Schedule in-person or virtual consultations with prospective clients at local area treatment centers, jails, DCS and Probation offices.

• Document inquiries appropriately and facilitate admissions with Evolve Admission team.

Essential Functions:

1. Respond within 30 minutes or drive time of the initial request for an onsite consultation.
2. Present neatly dressed and in a professional manner to referral sources.
3. Collect screening information for potential patients.
4. Possess working knowledge of chemical dependency signs and symptoms in adults and adolescents.
5. Demonstrate knowledge of appropriate emergency, indigent, and self-help referral information.
6. Maintain legible handwriting.
7. Exercise good judgment in providing clients with appropriate resource options.
8. Motivate potential clients who may be in denial of their situation.
9. Understand insurance and third-party payers.
10. Possess complete knowledge of crisis response role.
11. Demonstrate timeliness in executing the Mobile Intake Specialist position.
12. Obtain and document substance abuse history.
13. Coordinate screening and referrals with Evolve Admissions Team.
14. Maintain professional demeanor and appearance.
15. Work cooperatively with colleagues, managers, clients, and prospective clients.
16. Work in a constant state of alertness to perform job safely.
17. Know and abide by confidentiality regulations.
18. Other duties as assigned.

Primary Duties of Mobile Intake Specialist:

• Provide initial assessment and screening for clients interested in services through Bradford Evolve Treatment Services.
• Assist in planning and providing services to individuals with substance use and/or co-occurring mental health disorders.
• Collaborate with Clinical Director and clinical team.
• Help clients develop achievable goals and objectives within their Individual Success Plans.
• Coordinate with collaborating agencies and external service providers.
• Participate in meetings and trainings.
• Respond to crisis and emergency calls.
• Maintain professionalism and confidentiality.
• Travel between Evolve locations for intake needs.
• Complete a minimum of two intakes daily for potential incoming clients.
• Provide tours and introductions to services for new intakes.
