    Summary:
    We are seeking a dedicated and compassionate Qualified Intellectual Disabilities Professional (QIDP) to join our team. In this role, you will play a crucial part in developing and implementing Individual Habilitation Plans (IHP) for residents in our Department of Housing. As a QIDP, you will collaborate with an interdisciplinary team, engage with residents and their families, and ensure the successful execution of IHP goals.

    As a QIDP, you will be responsible for a range of essential duties, including but not limited to:

    • Investigating case situations and presenting relevant information to the interdisciplinary team.
    • Writing and implementing goals in the Individual Habilitation Plan for each resident.
    • Coordinating and scheduling IHP meetings with residents, family members, home managers, and other medical and social agency members.
    • Assisting residents in responding constructively to the IHP and making necessary adjustments to achieve set goals.
    • Serving as a link between residents, their families, and the community.
    • Monitoring staff to ensure the effective implementation of IHP goals.
    • Chairing IHP meetings and the Quarterly Drug Review Committee.
    • Initiating special meetings to resolve problematic situations with residents when necessary.
    • Reviewing goals of each resident quarterly and monitoring client participation in other programs.
    • Referring residents or families to other community resources.
    • Being on-call 24/7 to address any urgent situations.
    • Conducting investigations of alleged abuse/neglect and reporting findings to the Residential Director.
    • Visiting homes and reviewing programmatic binders monthly.
    • Participating in the Admission/Discharge process and completing related documentation.
    • Adhering to all deadlines related to essential job functions.
    • Knowing and carrying out established policies and procedures of the organization.
    • Attending ongoing training sessions.
    • Performing other routine duties as assigned by the Director of Residential.

    Supervisory Responsibilities:
    As a QIDP, you will work with supervisors overseeing residents in multiple homes. Your responsibilities include training employees on IHP implementation, planning, assigning, and directing work, and addressing complaints and resolving problems.

    Qualification Requirements:

    • Bachelor's degree (B.A. or B.S.) from a four-year college or university.
    • Two years of experience in a related field.

    You should possess:

    • Proficiency in developing Individual Habilitation Plans and Individual Treatment Plans.
    • The ability to deal with people in a courteous and tactful manner.
    • The ability to identify residents' needs.
    • The ability to conduct meetings professionally.

    Physical Demands:
    The role requires the ability to lift and/or move more than 100 pounds. Clients need to be lifted and/or moved from one position to another; this is not to be done without assistance and proper safety devices.
