Community Living Coordinator (DSP Supervisor)

Job Summary

Discovery Living has been serving adults with intellectual disabilities for over 40 years and sets the gold standard for service provision in the area of disability services. We are currently hiring for our Community Living Coordinator (CLC) role. As a CLC, you will oversee the service provision for 3 HCBS residential homes, supervising anywhere between 15 to 20 employees, with primary responsibility over roughly 15 adults with disabilities. The Community Living Coordinator is a management position responsible for the support services carried out by assigned Direct Support Professionals and Associate Coordinators (i.e., house leads), and is supervised by the Director of Operations.

Essential Functions

• Provides supervision and direction to direct support staff members.
• Evaluates work performance of each direct support team member.
• Participates in the screening and hiring of staff.
• Participates in staff training events.
• Attends agency management meetings.
• Monitors program compliance with Supported Living regulations.
• Compiles program evaluation data.
• Maintains complete and current member files.
• Coordinates medical and social services for members.
• Communicates with family members and other professionals on a regular basis.
• Provides informal counseling and promotes member self-advocacy.
• Assists members in managing personal finances.
• Coordinates implementation of member individualized plans.
• Actively participates in interdisciplinary team meetings.
• Assists members in obtaining other services available to them.
• Continuously evaluates health, safety, and security issues affecting members.
• Participates in member pre-admission screening.
• Participates in agency on-call schedule.
• Provides transportation in agency vehicles.
• Effectively monitor and manage agency financial resources.
• Perform other duties as requested.

Qualifications

A high school diploma or GED is required; post-secondary education preferred. A minimum of two years of professional experience in the ID/DP (intellectual disabilities/disable persons) field, with prior leadership experience, is required. Must have a valid Iowa driver’s license and be insurable through the Discovery Living Inc. insurance carrier.

• Must be able to adjust to and manage change, including crisis or unusual situations. Show an interest in acquiring new skills and be enthusiastic about performing the job to the best of their ability.
• Work diligently toward total community inclusion and encourage member choice. Believe that member empowerment is central to service delivery and create an atmosphere that promotes member decision-making.
• Because exact work hours will vary with the needs of the members, must be willing to work a flexible schedule. The schedule may vary from day to day and some weekend work may be required.

Job Type: Full-time

Pay: $50,000.00 - $55,000.00 per year
