Responsibilities of the Clinical Social Worker will include assessing a patient's psychosocial needs, developing individualized treatment plans, and providing external resources or referrals as needed. The Clinical Social Worker is also accountable for completing all written reports in a timely fashion, maintaining patient records as required by law and Medical center policy, and continuously cross-collaborate with multidisciplinary teams, case management, and external providers as needed.
