ACHC is a Federally Qualified Health Center and licensed primary care clinic. We provide Family Medicine and Dental Care, with additional specialists in Behavioral Health, Chiropractic, Internal Medicine, Neurology, Obstetrics and Gynecology, Pediatrics, Podiatry and Optometry. ACHC Clinics are located across Fresno, Kings and Tulare counties.
Position Summary
Provide therapeutic health services, counseling and case management for the patients including high risk and low-income individuals and families. The LCSW will be working as part of our multidisciplinary team with an emphasis on our chronic patients, with conditions such as diabetes, cardiovascular disease, substance abuse and depression. May include traveling to our various sites.
Essential Job Functions and Requirements:
Our therapists provide individual and group counseling services for adults, children and family therapy. This includes but is not limited to:
-Participating in interdisciplinary treatment planning
-Evaluating data to identify causes of problem of individuals and to determine advisability of counseling or may refer to other specialist or institutions
-Counseling or therapeutic interviewing to assist individual to gain insight into personal problems, define goals, and plan action reflecting interest, abilities and need
-Following up on results of counseling to determine reliability and validity of treatment used
-Documenting in medical record according to requirements of company and regulatory agencies
-Providing crisis intervention consultation
Other areas in which our therapists participate:
-Providing consultation/education to community service clubs, organizations, human services agencies, hospitals, and various county departments
-Program development and staff in-services
-Performing administrative functions consistent with the needs of the program
-Participating in staff meetings as required
-Meet monthly productivity requirement and assigned goals

Involvement in the Medication Assisted Treatment (MAT) Program
Our Medication Assisted Treatment Program assists people with attaining and maintaining recovery from opioid dependence. When appropriate, we prescribe Suboxone to provide Opioid dependence treatment for patients with chronic pain disorders or for patients whom are dependent on Opioids and have been chronically treated for pain with Opioids. This involves a team approach with the staff at Aria Community Health Center supporting each patient’s recovery or therapy for dependence or both through use of prescription medications, counseling, group sessions, case management services and other services to provide a whole patient approach. These treatments work together to assist patients in reducing the cravings and the other symptoms related to withdrawal. management of your substance use disorder and the responsibility to follow the agreed-upon treatment plan.
Minimum Qualifications
Master’s in Social Work from an accredited college or university and must be licensed as a Clinical Social Worker in the state of California.
Organized, self-driven, team player with technical skills.
An understanding of the cultural sensitivities in this type of work and the use of cultural appropriate methods is important.
Spanish speaking is preferred but not required.
Experience working with farm worker communities and under-served populations is a plus.
PHYSICAL REQUIREMENTS
Hearing: Adequate to perform job duties in person and over the telephone.
Speaking: Must be able to communicate clearly to patients in person and over the telephone.
Vision: Visual acuity adequate to perform job duties, including reading information from printed sources and computer screens.
Other: Requires occasional lifting and carrying items weighing up to 10 pounds unassisted. Requires frequent bending, reaching, and repetitive hand movements (specifically keyboarding and writing), standing, walking, squatting and sitting, with some lifting, pushing and pulling exerted regularly throughout a regular work shift.
