Dialysis Center of Lincoln is hiring a full-time Licensed Clinical Social Worker. Our Social Workers provide exemplary patient care as part of a dynamic interdisciplinary team. Social workers are advocates for our patients and engage with community providers to enhance our patients’ health and overall wellbeing. Our social workers build long -term, meaningful relationships with our patients and their families as well as other members of the interdisciplinary team.
Previous nephrology experience preferred, but not required.
GENERAL SUMMARY:
Social Worker provides psychosocial assessment, counseling, and resource referral services to Dialysis Center of Lincoln patients; works collaboratively with the interdisciplinary treatment team; assists in developing resources; and participates in patient/family and staff orientation and education. Social Worker must possess the physical capabilities to successfully perform the job as stated in the physical requirements. Delegation of duties is permissible, but the Social Worker remains the responsible party.
Adherence to all DCL policies and procedures is required.
EDUCATION AND EXPERIENCE REQUIREMENTS
1. Master’s degree in social work from a C.S.W.E. accredited school of social work.
2. Must be licensed in the State of Nebraska, or eligible for license.
(Supervision will be provided).
ESSENTIAL JOB FUNCTIONS:
JOB SPECIFIC TASKS:
- Complete a psychosocial assessment on dialysis patients at initiation of dialysis, 90 days, annually, and if determined to be unstable.
- Counsel chronic dialysis patients and their families. Assist patient/family in adjusting to treatment and altered lifestyle.
- Participate in team review of patient progress and recommend changes in treatment based on the patient's current psychosocial needs.
- Provide depression, anxiety, and cognitive screening with assessments and as indicated.
- Develop and record care plans to address mental health issues, psychosocial, and rehabilitation goals in the medical record for each dialysis patient and provide follow up.
- Inform patients of services and programs available through Nebraska Chronic Renal Disease Program, Nebraska Kidney Association, and all community and national resources.
- Possess working knowledge of literature and research pertaining to psychosocial aspects of renal disease and share this appropriately with team members.
- Participate in professional organizations and conferences in developing and enhancing services for renal patients. Attend appropriate continuing education institutes and conferences per administrative approval to keep abreast of research/literature/developments in nephrology social work.
- Provide consultation/orientation for new employees and groups as requested.
- Establish and maintain rapport and working relationships with patients, families, and other members of the health care team.
- Attend and participate in patient care conferences.
- Attend and participate in employee meetings and job-related in-services.
- Maintain membership in and attend Nebraska Council of Nephrology Social Worker meetings.
- Organize workload and set priorities in a logical and systematic way.
- Maintain an awareness of, and participate appropriately in development of norms, standards, and criteria for Social Work Services.
- Responsible for own actions and seeks supervision when indicated.
- Know, interpret, and adhere to policies, procedures and practices of the Dialysis Center of Lincoln, Inc.
- Travel within Nebraska required once a week- rental car/mileage reimbursement provided.
- Perform other related work as assigned.

Actions are important in maintaining high quality patient care. Good judgement is a very essential part of this position. The health of patients depends upon the judgement of the clinical social worker. Work is subject to checks and controls.

DEPENDABILITY:
Providing quality dialysis to our clients is dependent on the conscientious, timely attendance of all staff according to their schedule, including taking calls, if applicable. It is expected that there will be no more than six incidences/year.
TEAMWORK:
Teamwork is essential for a good work environment at the Dialysis Center of Lincoln. All staff are expected to support each other. Conflicts arising between staff are expected to be worked out in a professional manner.
COMMUNICATION:
Clear verbal and written communication skills are essential when dealing with dialysis patients, families, staff members, physicians, vendors, hospital personnel and other outside sources.
CONTINUOUS IMPROVEMENT:
Supports and participates in continuous quality improvement looking for ways to improve processes in a cost-effective manner. Identifies and communicates quality improvement measures to others.
SAFETY:
Because there is possible contact with infectious diseases, good safety and personal sanitation habits are important. Some exposure to environmental hazards is possible. Caution must be exercised to prevent injury. Injuries could be serious.
PROFESSIONAL AND PERSONAL GROWTH:
Regular in-services as well as staff meetings are provided on a regular basis.
NON-ESSENTIAL JOB FUNCTIONS:
1. Participates on committees as requested.
2. Performs all other related duties as assigned within the employee's physical and mental capabilities.
PHYSICAL REQUIREMENTS:
ESSENTIAL:
1. Position may require up to 75% of time spent being mobile within the Dialysis Center of Lincoln.
2. Hearing required.
NON-ESSENTIAL:
1. Able to lift up to 25 pounds.
2. Manual dexterity is helpful when utilizing computers for inputting patient information.
