Qualifications
Responsibilities:
- Perform initial evaluation and ongoing physical therapy re-evaluations according to APTA standard of practice as well as state guidelines
- Develop a Plan of Treatment and coordinate with the referring physician, as well as the patient and/or patient caregiver
- Implement the Plan of Treatment on a consistent schedule and modify according to patient progress.
- Evaluate patient mobility, strength, and level of function to perform ADLs, and set attainable goals for the patient
- Maintain confidential and accurate patient treatment records
- Develop an At-Home exercise plan with consideration to the patient’s living and social situation, and at the completion of treatment, discharge the patient when appropriate with an At-Home plan of care.
- Maintain all professional licensing per federal and state guidelines, as well as credentialing with all major insurance payers.
- Maintain professional continuing education requirements through participation in on-going training seminars, professional workshops, etc.
