Key Responsibilities:

• Assist Physical Therapists in conducting assessments and developing individualized care plans for patients.
• Provide hands-on, one-on-one physical therapy sessions under the supervision of licensed therapists.
• Collaborate with the healthcare team to ensure comprehensive patient care.
• Educate patients and their families on exercise routines and home care techniques.
• Maintain accurate patient records and document progress.

Qualifications:

• Valid Physical Therapist Assistant (PTA) license in the state of Illinois.
• Strong clinical skills and a commitment to providing exceptional patient care.
• Compassion, empathy, and excellent communication skills.
• Reliable transportation and willingness to travel to patients' homes.
• Experience in home health care is a plus but not required.
