Responsibilities

Our LPN in Home Care provides skilled nursing care in the patient’s home appropriate to the adolescent, adult and geriatric patients served. Under supervision of a Registered Nurse, the LPN provides quality care to patients and performs technical skilled care in patient’s home.

Essential Functions:

• The LPN possess and practices appropriate physical assessment skills, including assessment of the adolescent, adult and geriatric patient, family and home situation.
• Provides ongoing assessment of patient’s condition and nursing needs.
• Prioritizes patient’s problems or needs according to urgency to reflect the physical, psychological, emotional and spiritual aspects of care of the patient.
• Completes OASIS assessments per Medicare guidelines.
• Fulfills on call responsibilities when assigned.
• Acts as a case manager to coordinate all home care services. Supervises Home Health Aides.
• Clarifies and implements physician orders and nursing care plan according to established standards.
• Performs medication reconciliation and instructs patient/family in medication use and precautions.
• Develops and revises an individualized plan of care 
• Documents according to departmental standards.

Grand View Health's Home Care department serves Bucks and Montgomery communities. GVH Home Care strives for excellency by assisting patients with their health needs in the comfort of their home. The Home Care staff is expected to commute within a 15-mile radius of Grand View Hospital. Gas mileage will be reimbursed.  

Qualifications

Education and Experience:

Vocational diploma Licensed Practical Nursing Diploma, required 1 year to 3 years Nursing Experience, required

License and Certifications:LPN in PA – Licensed, required upon hireBasic Life Support (BLS) – required upon hireNon- Clinical Drivers License – PA – required upon hireAutomobile insurance coverage – required upon hire
