Licensed Practical Nurse - Home Care administers nursing care in a home care environment under the supervision of a registered nurse or other medical supervisor. Provides basic medical care, including changing bandages, administering medication, and collecting specimens. Being a Licensed Practical Nurse - Home Care ensures the health, comfort and safety of patients by assisting with bathing, feeding, and dressing. Monitors and reports changes in patient's condition to supervisor. Additionally, Licensed Practical Nurse - Home Care requires a high school diploma. Typically reports to a registered nurse or supervisor. Requires graduation from approved LPN educational program. Requires a state license to practice. Licensed Practical Nurse - Home Care's years of experience requirement may be unspecified. Certification and/or licensing in the position's specialty is the main requirement. Responsibilities for Licensed Practical Nurse (LPN)
- Perform home visits to assess the patient’s health needs and implement a nursing care plan.
- Responsible for delivering a quality medical care in compliance with industry/company regulations, policies, and procedures.
- Administering medications to patients and monitoring patients for reactions or side effects.
- Monitoring all aspects of patient care, including diet and physical activity.
- Modifying patient treatment plans as indicated by patients' responses and conditions.
- Provide education to patients and families on proper home health care procedures and strategies.
Qualifications for Licensed Practical Nurse (LPN)- Proven record of providing patients with nursing care.
- Ability to exercise independent and sound judgement.
- Strong decision-making skills and well-developed interpersonal skills.
- Effective organizational and time management skills along with strong teamwork skills.
- Experience with a wide range of patient age groups and disorders.
Experience and Education for Licensed Practical Nurse (LPN)- Experience in providing nursing care to patients in a residential setting or similar environment.
- Current Licensed Practical Nurse license for the state of operation.
