Licensed Practical Nurse - Home Care administers nursing care in a home care environment under the supervision of a registered nurse or other medical supervisor. Provides basic medical care, including changing bandages, administering medication, and collecting specimens. Being a Licensed Practical Nurse - Home Care ensures the health, comfort and safety of patients by assisting with bathing, feeding, and dressing. Monitors and reports changes in patient's condition to supervisor. Additionally, Licensed Practical Nurse - Home Care requires a high school diploma. Typically reports to a registered nurse or supervisor. Requires graduation from approved LPN educational program. Requires a state license to practice. Licensed Practical Nurse - Home Care's years of experience requirement may be unspecified. Certification and/or licensing in the position's specialty is the main requirement. STAT Home Health iis seeking Part-Time Licensed Practical Nurses (LPN) to service patients in Destrahan, LA!

Essential duties include:
- Provide nursing and supportive care in accordance with the Interdisciplinary Plan of Care, under the direction of the RN
- Communicate with the attending physicians, other home health team members, and other agencies as needed to coordinate optimal care and use of community resources for the patient/family.
- Document all services provided.
- Demonstrate knowledge of Medicare/Medicaid regulations for home health benefits.
- Report changes in patient condition and needs to RN
Qualifications:
- Current Louisiana Licensed Practical Nurse (LPN) license required
- Minimum 1 year clinical experience as an LPN
- Current CPR Certification
- Compassionate and great time management skills
- Reliable transportation to conduct patient visits at their place of residence or facility
