Licensed Practical Nurse - Home Care administers nursing care in a home care environment under the supervision of a registered nurse or other medical supervisor. Provides basic medical care, including changing bandages, administering medication, and collecting specimens. Being a Licensed Practical Nurse - Home Care ensures the health, comfort and safety of patients by assisting with bathing, feeding, and dressing. Monitors and reports changes in patient's condition to supervisor. Additionally, Licensed Practical Nurse - Home Care requires a high school diploma. Typically reports to a registered nurse or supervisor. Requires graduation from approved LPN educational program. Requires a state license to practice. Licensed Practical Nurse - Home Care's years of experience requirement may be unspecified. Certification and/or licensing in the position's specialty is the main requirement. Current Louisiana Licensed Practical Nurse (LPN) license required Minimum 1 year clinical experience as an LPN Current CPR Certification Compassionate and great time management skills Reliable transportation to conduct patient visits at their place of residence or facility
Job duties:Provide nursing and supportive care in accordance with the Interdisciplinary Plan of Care, under the direction of the RN Communicate with the attending physicians, other home health team members, and other agencies as needed to coordinate optimal care and use of community resources for the patient/family.
Document all services provided. Demonstrate knowledge of Medicare/Medicaid regulations for home health benefits.
