Licensed Practical Nurse administers nursing care under the supervision of a registered nurse or other medical supervisor. Provides basic medical care, including changing bandages, administering medication, and collecting specimens. Being a Licensed Practical Nurse ensures the health, comfort and safety of patients by assisting with bathing, feeding, and dressing. Monitors and reports changes in patient's condition to supervisor. Additionally, Licensed Practical Nurse requires a high school diploma. Typically reports to a registered nurse or supervisor. Requires graduation from approved LPN educational program. Requires a state license to practice. Licensed Practical Nurse's years of experience requirement may be unspecified.   •  Purpose of PositionProvides nursing care and treatment to patients in their homes under the supervision of a registered professional nurse.

    Responsibilities:

    1. Provides direct patient care and instruction to the patient/family according to the Plan of Treatment within the scope of the LPN License and Practice. This includes but is not limited to administering subcutaneous and IM injections, wound care, vital signs, venipuncture, tracheotomy care and cauterizations.
    2. Assists the patient to participate in care with the ultimate goal of independence with all activities of daily living.
    3. Participates in the planning, implementation and evaluation of nursing care (attends case management conferences)
    4. Documents on a timely basis, all care and instruction provided to patients/families, and maintains records and reports according to Agency policy.
    5. Educates the patient/family/caregiver regarding the disease process, self-care techniques and prevention strategies
    6. Maintains service productivity to meet Agency goals and objectives.
    7. Reports significant conditions/symptoms to the patient’s physician and VNSW Case Manager
    8. Maintains confidentiality of patient records as per Agency policy.
    9. Demonstrates a knowledge of and adherence to Agency policy as well as state/federal regulations governing home care services.
    10. May accept verbal orders from a Physician and carries out orders.
    11. Participates in activities to enhance professional growth

    Qualifications:

    1. Graduate of NYS approved program for Licensed Practical Nurse.
    2. License and current registration to practice as a Licensed Practical Nurse in the State of New York
    3. At least two years of experience in medical/surgical nursing and/or previous homecare experience.
    4. Professional Liability and automobile insurance in the amount specified by the Agency.
    5. Automobile in good working order and available for use in the field
