Licensed Practical Nurse - Long-Term Care administers nursing care in a long-term care environment under the supervision of a registered nurse or other medical supervisor. Provides basic medical care, including changing bandages, administering medication, and collecting specimens. Being a Licensed Practical Nurse - Long-Term Care ensures the health, comfort and safety of patients by assisting with bathing, feeding, and dressing. Monitors and reports changes in patient's condition to supervisor. Additionally, Licensed Practical Nurse - Long-Term Care requires a high school diploma. Typically reports to a registered nurse or supervisor. Requires graduation from approved LPN educational program. Requires a state license to practice. Licensed Practical Nurse - Long-Term Care's years of experience requirement may be unspecified. Certification and/or licensing in the position's specialty is the main requirement. Job Summary
Provides direct patient care under the direction of a registered nurse, nurse practitioner, physician assistant, or physician. Training is provided relevant to the population served, based on scope of care of the service assignment.Job Responsibilities- Contributes to the assessment of patients by collecting data for review.
- Administers medications and observes patients for adverse reactions.
- Performs plan of care interventions.
- Evaluates learning needs of patients and families and provides education based on age, culture, and willingness to learn.
- Performs job duties in accordance with the Nurse Practice Act.
Qualifications- High school diploma, General Equivalency Diploma (GED) or higher required upon hire AND
- Associate's degree or higher in Nursing required within 5 years of employment in this position

- LPN - Licensed Practical Nurse and eligible to practice in the state of Kentucky required upon hire AND
- BLS - Certified as a Basic Life Support Healthcare Provider required within 45 days of employment in this position AND
- RN - Licensed as a Registered Nurse. Eligible to practice nursing in the State of Kentucky required within 60 months of employment in this position
