Licensed Practical Nurse - Long-Term Care administers nursing care in a long-term care environment under the supervision of a registered nurse or other medical supervisor. Provides basic medical care, including changing bandages, administering medication, and collecting specimens. Being a Licensed Practical Nurse - Long-Term Care ensures the health, comfort and safety of patients by assisting with bathing, feeding, and dressing. Monitors and reports changes in patient's condition to supervisor. Additionally, Licensed Practical Nurse - Long-Term Care requires a high school diploma. Typically reports to a registered nurse or supervisor. Requires graduation from approved LPN educational program. Requires a state license to practice. Licensed Practical Nurse - Long-Term Care's years of experience requirement may be unspecified. Certification and/or licensing in the position's specialty is the main requirement.     Responsibilities

    • Performs duties within scope of license and within care plans.
    • Works closely with care team on providing exceptional care and services; provides adequate communication to the team responsible for the patient.
    • The LPN will provide skilled nursing care in the home as ordered on the plan of care and under the supervision of the RN. 
    • Performing routine levels of care. 
    • Performs specific treatments and administers medications as ordered.
    • Performs duties using aseptic and safety measures as trained.
    • Educates patients and family regarding the patient’s illness, procedures, treatments, medications, also including self-care, prevention and health maintenance; provides appropriate age specific education and communication. Documents all changes in patient’s physical or psychosocial condition, immediately notifying RN of any significant changes.
    • We use EPIC for EMR, position requires computer skills for accurate and concise documentation.

    Preferred Qualifications

    • Homecare experience a plus! 
    • Positive experience with complex patients with dementia, Alzheimer’s disease, COPD, cancer and joint/hip replacements.

     Required

    • Registered as Licensed Practical Nurse with New Hampshire’s Board of Nursing.
    • CPR Certification.
    • Satisfactory criminal record.
