Licensed Practical Nurse - Nursing Home jobs in Cleveland, OH

Precious Home Healthcare LLC is seeking full-time and part-time Licensed Practical Nurses to join our team in providing exemplary skilled nursing care to patients on an intermittent basis in their homes. All care is provided in accordance with physician orders and plan of care under the direct supervision of the Registered Nurse. Work hours may vary based on the needs of the assigned patients, and nights, weekends, and holidays may be worked as assigned.

The role of the LPN is to ensure that nursing services, treatments, and diagnostic and preventive procedures are provided in accordance with the nursing care plan. LPNs will also support their patients by teaching and counseling them and their families on topics related to nursing care or other needs of the patient at home. This can include teaching patients on appropriate self-care techniques. In addition, responsibilities include:

• Observing signs and symptoms and reporting to the physician and RN reactions to treatments, including drugs and changes in the patient's physical or emotional condition.
• Maintaining accurate and complete records of observations, treatments and care of patient.
• Submitting any changes in schedule to Director of Clinical Services/Nursing Supervisor on a daily basis.
• Participating in patient care conferences to discuss the need for involvement of other members of the health team, such as physical therapist or speech language pathologist.
• Assisting the physician and RN in performing specialized procedures.
• Preparing equipment and materials for treatments.
• Observing aseptic technique as required.

To be qualified for this position, you must be a graduate of an accredited school of practical/vocational nursing and currently be licensed in the state of Ohio. One year of nursing experience is preferred. Because you will be providing healthcare to patients in their homes, reliable transportation is required. Because some patients may need support turning or being repositioned, this position does require a level of physical activity including heavy lifting, extensive bending, and standing.

All applicants must be able to pass a BCI background check and two-step PPD Tuberculosis test at the applicant's expense. Proof of CPR certification is required.
