Position Summary: The Licensed Practical Nurse will provide personal care services, assistance, support and supervision to residents. The nurse will administer routine medications to residents in accordance with state regulations, physician orders, and service agreements with Registered Nurse supervision. Use superior assessment and communication skills. Candidates will have the ability to enrich resident’s lives by addressing their physical, intellectual, emotional, and social well-being. Other duties as assigned.

Essential Job Responsibilities
• Work with team to update service plans for residents.
- Assist in ADLs, administer medications, treatments, and services as indicated on the resident service plan
- Document resident information pertaining to changes in residents' functional statuses, personal needs, appointments, etc.
- Administer medications as directed in the resident service plan and MAR.
- Administer medications accurately and at appropriate times according to physician orders.
- Document medications administered and those not given according to regulations and policies and procedures.
- Administer treatments as directed on treatment sheets.
- Document information pertaining to changes in residents’ functional statuses, personal needs, appointments, etc.
- Communicate resident needs, conditions, and changes with families
- Perform vital signs, and weights as needed
Education, Experience, and Other Requirements
- LPN nursing license in the state
- CPR certification required
- Basic First Aid Training
- Experience in working with older adults
Knowledge, Skills, and Abilities Required
- Promote teamwork in providing services to residents.
- State and Federal Guidelines for Assisted Living
- Nursing standards of practice for older adults
- HIPAA requirements
- Safe food handling procedures
- Ability to organize and prioritize with good attention to detail
- Intermediate knowledge or willingness and ability to learn the computer programs used in this community.
Preferred Qualifications
- OSHA-MSDS sheets and procedures
- Basic computer skills
- Previous experience in senior living
- Previous memory care experience preferred
