Evergreen Commons Rehabilitation and Nursing has openings for some great, energetic and passionate Licensed Practical Nurses (LPNs) to assist with resident care in our sub-acute rehab and skilled nursing facility. We currently have Full-Time, Part-Time-all shifts are available!

The Licensed Practical Nurse- LPN provides nursing care to residents as prescribed by the physician and in accordance with standards of nursing practice and regulations and as directed by the Director of Nursing.

Responsibilities:

• Collecting information from the residents to be admitted; medical records, insurance details.
• Recording health details of residents; including vitals & temperature.
• Administering medications and injections to residents as needed.
• Treating and dressing wounds and bedsores as needed.
• May be required to supervise Certified Nursing Assistants (CNA's).
• Helps residents get dressed and take care of personal hygiene.
• Monitors residents' food and liquid intake and output.

Requirements for Licensed Practical Nurse (LPN):

• Valid LPN License
• Must be in good standing with the State Registry
