The Case Manager is responsible for tracking homeless guests’ progress through case management who are enrolled in the various grant programs administered by CityLink. Ensure timely documentation of services (including case files, client outcomes, and agency technical reports) and timely data input into County HMIS system. Working within the Housing First best practices and collaboration with the Continuum of Care (CofC) and Consolidated Entry System (CES). This position is paid by grant funds and is, therefore, contingent upon the availability of grant funds. Additional future grants are probable.
Provide case management for homeless guests who qualify for financial assistance through the various grants administered by CityLink. Also provide case management for homeless guests who are being served across the Pathway to Housing program as necessary. Conduct initial intake coordinating eligibility status with 211 United Way via the Coordinated Entry System when applicable. Schedule appointment with guest to initiate income eligibility and gather all required documentation for case files to determine financial assistance. Once eligibility is determined, establishes the case file, guest’s Individual Housing Action Plan (IHAP) and identifies wrap around support services that are needed for stabilization and sustainability. Track guest goals, milestones and outcomes, which may require home visits. Answer email and phones, maintain calendar, and maintain case files for grant monitoring ensuring privacy of all guests PII and that all records are properly secured. Assist with preparing materials for program using a variety of methods, including Word, PowerPoint, Excel and Publisher. Ensure that current policies and procedures and best practices are followed.
Enter all guest information into HMIS database by ensuring that information is entered accurately verifying by producing technical reports, which will be reviewed by Grant Manager. Monitor use of computer station ensure use by authorized personal only.
Work with Street Outreach Specialist, CAP Coordinator and other Case managers to build and leverage resources for guests seeking wrap around supportive services from CAP/CityLink who may qualify for grant financial assistance. Assist with class schedule, set-up and record keeping. Ensure that current policies and procedures for intake and distribution of services are consistent across all programs. Attend case conferencing meetings. Participate in all CityLink outreach events that support wrap around services such as: CEO’s in the Park, Holiday Food Baskets and Christmas Toy Store. Other duties as assigned.
