Assertive Community Treatment (ACT) was created in the 1970s and is one of the oldest and most widely researched evidence-based practices working with severe mental illness adult clients, ages 25 and older. By providing person centered assistance including housing, recovery, vocational support, nurse/psychiatrist and therapist services, we enable clients to eventually transition to lower levels of care programs or move toward independence, increasing their quality of life. Clients may include adults with a history of trauma, homelessness, frequent hospital stays, refugees, and clients who have not responded well to traditional outpatient care.
About your Role:
Our Vocational Specialists work as a member of the ACT team, advocating and working with your assigned adult clients. The team is comprised of several members, each with their own focus with the client, including housing, vocational services, therapy, medical, and substance use. As our Vocational Specialist, you will focus on client’s employment goals, assessing the effect of their mental illness on employment, and engaging the client in creating and implementing an Individual Recovery Plan (IRP). This IRP will focus on the client’s self-articulated goals and objectives, recovery risk factors and relapse prevention ideas, strengths, personal and employment barriers, emotional triggers, and their personal definition of recovery. You will create a strategy based on the client’s employment interests, skillsets, and challenges and provide training, support, and skills to aid the client in gaining and maintaining employment. This position will educate them on financial management, the pros and cons of disclosing disability aspects to employers, the impact of increased wages on eligibility and disability, housing, or other income determined services. As the client’s advocate, you will link them to necessary services in the community to assist with their goal of employment. You will further assist with pro-active crisis intervention and a job support plan to prepare them for continued employment in this role. This position includes client transportation as needed and one week on-call rotation every 6 weeks, handled virtually or in person, depending on the situation. For optimal client success, you will meet with clients in the local community primarily.
