Job Title: Case Mgr/Discharge Planner/Utilization Review

Location: Meeker, CO

Guaranteed Hours: 40

Shift - M-F 8-5

Number of Weeks: 13

Facility is also accepting Full time position for this.

Seeking energetic, capable, skilled case manager with knowledge of IMM, MOON, HINN notices and experience with MCG. Must have computer skills

HOUSING: Possibility of sharing 2 bedroom/2 bath apartment

Responsibilities

• Utilize knowledge of DRG, EMR systems, and EHR systems to facilitate efficient discharge planning processes.
• Collaborate with healthcare team members to ensure timely and appropriate patient care transitions.
• Maintain accurate medical documentation and records in compliance with HIPAA regulations.
• Apply medical terminology and physiology knowledge to support patient care needs.

Utilize premium skills such as Epic and Managed care to enhance discharge planning strategies.

Job Type: Contract

Pay: $75.00 - $80.00 per hour

Expected hours: 40 per week
