The Clinical Monitoring and Review Coordinator is responsible for working collaboratively with the ANS Nursing team and the patient’s pharmacy to provide seamless patient and referral source care through quality and coordination of care. The role collaborates with the treatment teams to coordinate documentation requirements and deliverables. The position is responsible for reviewing patient documentation and ensuring the information is received in a timely manner. The coordinator is also responsible for periodic quality audits, data mining, initiating, and supporting processes for the team. The coordinator will assist with printing and shipping of patient welcome and admission packets to field nurses.
Office Hours: Monday through Friday from 8:30am to 5pm
EDUCATION AND EXPERIENCE:
· Preferred: Licensed Practical Nurse (LPN), in good standing with the practicing state’s Board of Nursing, with at least 1 year of acute care or specialized alternate site IV therapy experience preferred
· Alternate: Medical Assistant in Home Health or Hospital setting with at least 1 year of experience.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
• Works collaboratively with nurse manager and field nurses to ensure EMR database is maintained and processes are followed.
• Reviews and audits patient medical charts for accuracy, audits, monitors and insures completion of documentation.
• Works collaboratively with all members of the nursing team including the Administrator and Governing Body to ensure the patient care documentation, data collection, and coordination of care is completed. Ensures the documentation is accurately maintained and processes are followed.
• Ensures quality customer service.
• Participates in nursing multidisciplinary meetings.
• Maintains patient records in compliance with HIPAA, state and ACHC/URAC regulations.
• Participates in performance improvement activities for the enhancement of care delivery and offers suggestions and assistance towards the continuous quality improvement of services offered to consumers.
• Maintains knowledge of state specific regulatory guidelines that impact patient care. Supports RN Regulatory Manager is tasks related to home health or accreditation tasks.
• Recognizes patients’ rights and responsibilities and supports them in the performance of job duties; respects patient’s rights to privacy and confidentiality.
• Represents self in a professional and ethical manner at all times.
• Complies with all applicable company policies and procedures.
• Stays knowledgeable of current clinical issues and standards and incorporates them into practice.
• Interacts with others in a positive, respectful, and considerate manner.
• Performs other job-related duties as assigned.
