Exciting opportunity for a Bilingual (Spanish/English) Bachelor Level individual to work for Mid-Fairfield Community Care Center in Norwalk, CT to provide care coordination services to children and their families. Position will be working with primarily Spanish-speaking families.

Position is a full time, 40 hour position, Monday through Friday. Schedule is flexible but typically involves 2 later days 11-7:00 pm or 12-8:00 pm
Qualifications:
The Community-Based Care Coordinator will work in the schools, in-homes, and/or in the community providing extensive care coordination to help support students and their families.
The Bilingual Community-Based Care Coordinator will help families access a range of community services and resources and strengthen the ability of families within greater Norwalk to meet the multiple needs of their children. This position requires that the candidate be comfortable with initiating active community outreach to maintain and establish resources, public speaking and group facilitation. A fluent speaker in Spanish and English is required. Excellent opportunity for individuals to learn more about the mental health field working with therapists, psychiatrists and other community health partners.
