Managed Care Coordinator jobs in Twin Falls, ID

Innovative Health Care Concepts (IHCC) is seeking energetic, self-motivated individuals for our Targeted Service Coordination program in our Twin Falls office.

This is a professional level position working closely with people with developmental disabilities. Our current opening is part-time with an opportunity to grow.

Applicant must have at least a Bachelor's degree in a human service field. Previous experience working with individuals with developmental disabilities is preferred but not required.

Job duties:

• Coordinate and oversee service delivery for individuals with developmental disabilities;
• Develop and implement individualized service plans;
• Collaborate with clients, families, and support teams to ensure effective service delivery;
• Monitor and evaluate services and make necessary adjustments to service plans;
• Provide crisis support as needed;
• Maintain accurate documentation and records.

Preferred skills:

• Relationship management: ability to build and maintain positive relationships with clients, families, and support teams;
• Social work: knowledge of social work principles and practices to effectively support clients;
• Administrative skills: strong organizational skill and attention to detail for maintaining accurate records.
