The Home Care Community Relations-Case Manager will be responsible for building and maintaining relationships with professional referral sources, clients and families in the North Shore area, while also developing new business opportunities.

- Excellent organizational and time-management abilities
- Knowledge of the North Shore area and its healthcare landscape
- Valid driver's license and reliable transportation
