Walnut Creek Senior Living is looking for your expertise to become our Hospitalist/Liaison for our long term care community which includes our three, elegant facilities. The role of the Hospital Liaison is to drive an ongoing increase in census results for our Walnut Creek, Greenbriar and Hawthorn Glen campus. This new key role will focus on generating new business and relationships for our facilities through a working collaboration with our local hospitals, physicians and hospital MSW contacts identified as a result of your resourcefulness.

Your previous success and expertise will enable you to:

• Nurture relationships with local area hospitals (administrators, physicians, social workers, and discharge nurses) to aggressively secure referrals for patients requiring placement in our nursing homes, assisted living facilities, memory care units, rehab, respite care options and our independent living environments.
• Keep focused and on target for hospital referrals by maintaining dedicated key relationships with established resources
• Aggressively work potential referrals for admissions to our network
• Be communicative with each facility with any new and anticipated potential admissions and associated updates coordinating marketing efforts for census growth
• Maintain professional relationships establishing trust with all communication to key personnel in our target communities

A proven track record with a minimum of three years of marketing experience in a clinical field preferably in a long term care environment will be required. Clinical experience is a plus in this role which helps to prove your credibility and will ensure continued success in driving referrals now and in the future.
